About Your Visit

As a neurologist, my office specializes in assessing and managing patients with suspected neurological disorders. To facilitate the visit, kindly complete the patient information form. If you are coming for memory or cognitive impairment evaluation, please fill out the attached form. For patients scheduled for assessment of numbness and weakness in limbs or nerve conduction testing, it is advised to take a shower beforehand and refrain from using lotions or oils. Keep arms and legs warm, even on warm days, by wearing extra clothing like gloves and stockings. Additionally, remember to bring a list of medications or the actual medications you are currently taking.
